Collaborate With Us

We are seeking a compassionate, motivated, and adaptable nurse to provide professional nursing services as an independent contractor within our Indigenous-led Lower Limb Preservation Team. This innovative mobile healthcare program delivers culturally safe, community-based lower limb preservation and wound care services to First Nations communities and urban Indigenous populations throughout Southwestern Ontario.

This is an exciting opportunity to collaborate with a small, interdisciplinary team dedicated to reducing preventable amputations, improving access to care, and supporting Indigenous health sovereignty through relationship-based care. 

Contract Overview

 The successful contractor will provide mobile nurse services, with most patient care delivered through home visits and community-based assessments. Working collaboratively with and reporting clinically to the physician lead. The contractor will help coordinate and deliver evidence-informed lower limb preservation and wound care services while building trusting relationships with patients, families and communities.

This independent service contract is anticipated to average approximately 20 hours per week; however, service hours may fluctuate depending on the program requirements and patient needs. Scheduling is highly flexible and will be determined through mutual agreement.

The successful contractor is responsible for maintaining all professional registrations, licences, certifications, liability insurance and other requirements necessary to provide services under this agreement.

Location of Services

This is a mobile, community-based contract requiring travel throughout Southwestern Ontario.

The primary service area includes:

• Chippewas of the Thames First Nation
• Oneida Nation of the Thames
• Munsee-Delaware Nation

Services may also be provided in London and surrounding communities as required to meet patient needs.

Regular travel is an essential part of this contract.

Scope of Services

The contractor will be responsible for providing professional nursing services, including but not limited to:

• Conduct comprehensive patient, lower limb, and wound assessments.
• Assess, treat, and monitor acute and chronic wounds.
• Develop and implement individualized wound care plans in collaboration with the clinical team.
• Provide parent and caregiver education to support healing and self-management.
• Document assessments and care accurately using approved electronic medical record systems.
• Coordinate and order wound care supplies and maintain familiarity with the available wound care products and vendors.
• Collaborate with community partners, home care providers, primary care teams, and other healthcare professionals.
• Assist with care coordination to ensure timely access to services and follow-up.
• Participate in quality improvement and program development initiatives, as appropriate.
• Travel regularly to patients' homes and community locations throughout the service region.

Required Qualifications

Applicants must possess:

• Current registration and good standing with the College of Nurses of Ontario as one of the following.
  • Registered Practical Nurse (RPN)
  • Registered Nurse (RN)
  • Nurse Practitioner (NP)
• Professional liability insurance appropriate to their scope of practice.
• Valid Ontario driver's license.
• Access to a reliable vehicle with appropriate insurance for work-related travel. 
• Ability and willingness to travel throughout the service region to provide home- and community-based care.
• Excellent assessment, communication, organization, and time-management skills.
• Ability to provide services independently while collaborating effectively within a small interdisciplinary team.
• Strong computer skills, including proficiency with electronic medical record system and other healthcare technologies.
• Commitment to culturally safe, patient-centred care.

Preferred Qualifications

Preference will be given to applicants with experience, education, or interest in one or more of the following:

• Wound care assessment and management.
• Lower limb preservation.
• Diabetic foot assessment and care.
• Compression therapy and/or Certified Compression Garment Fitter designation.
• Home and community nursing.
• Working with First Nations communities and an understanding of Indigenous health, cultural safety, and social determinants of health affecting Indigenous Peoples.

Equivalent combinations of education, experience, and willingness to learn will be considered. Clinical mentorship and orientation will be provided on specialized areas including wound care, lower limb preservation, foot care, compression therapy, and program-specific clinical protocols.
