NOTE: Additional information may be required at your next appointment. This form must have a completed “Client Consent and Release of Information” form attached to be valid.

Instructions

  1. Download the client intake form.
  2. Complete & sign the form on your computer, or print it out and complete by hand.
  3. Submit your completed form and resume to G.R.E.A.T.:

Please note that it may take up to 2 business days for a response once you have remitted your intake form, and up to 5 business days beyond the intake date for an appointment with an Employment and Training Coach. If your funding request has been approved by your ETC and all of your documentation has been submitted to them, please allow for 3 weeks processing time of your requests.
