ADVOCACY PREVENTION & SERVICE COORDINATION WORKER (APSW) Ref.#: 2025-45

(funded by the MCCSS as Adult Protective Services Worker)

Part-time (28 hrs/week), Permanent, Salaried

 

Salary: $48,592 - $54,665 - $60,739

To provide supportive counselling, advocacy, and service coordination for ‘at risk’ adults with a developmental disability, living in Haldimand & Norfolk Counties, under the direction of the Developmental Services Manager.

 

Responsibilities:

· Offer support, strategies, and inform individuals of choices that respond to their unique, individual needs that specifically relate to community life, independent living and general well being

· Ensure services involved in supporting a person focus on the person’s goals

· Ensure all necessary funded and non-funded services have been explored and person has been informed and educated on available resources and services

· Ensure regular communication with person and current service providers (with consent) to be current on status of services in person’s life (i.e., participating in support circles or life planning meetings)

· Participate in transitional planning with youth leaving the educational system

· Liaise and collaborate regularly with service providers with the goal of providing seamless service to individuals/families, based on a systems approach.

 

Qualifications:

· Successful completion of a university degree program in the Social Sciences from an institution of recognized standing or an acceptable equivalent

· Several years’ work-related experience in the adult developmental services field with particular skills developed in the areas of the assessment of needs, case management, advocacy, and community service planning

· Knowledge of regional services and local Haldimand-Norfolk services which support the target population

· Ability to problem-solve, think creatively and act quickly to meet families’ needs and deal with crisis situations

· Emotional maturity, tact, patience with ability to obtain confidence and develop relationships with children, adults, parents and professionals

· Excellent verbal & written communication and organizational skills

· Knowledge of Microsoft Office programs and data base experience

· Criminal & Judicial Matters and Vulnerable Sector Record Checks, Children’s Aid Society Declaration/Release

· Possession of a valid driver’s licence, $2 million vehicle liability insurance, clean Driver Abstract and use of a vehicle.
