JOB SUMMARY: The Private Personal Support Worker (PSW) will report to and work under the direction and supervision of the Home & Community Care Case Manager & the Private Client. The Private PSW will be assisting 48-year-old female Quadriplegic with all activities of daily living.
We currently have a strong group of caregivers but need to add a few more staff to the team. Experience is an asset but not required. The job involves personal care, meal preparation, & housekeeping. Additional duties as directed.
Self-Scheduling Shifts in a non-smoking household.

POSITION DESCRIPTION – HEALTH SERVICES
POSITION TITLE: Personal Support Worker
LOCATION: Private Client Home
REPORTING RELATIONSHIP:
Reports to and works under the direction and supervision from Private Client with additional collaboration from Home & Community Care Case Manager.
PURPOSE & SCOPE OF THE POSITION:
To assist Private Client in the performance of statutory, operational and advisory service duties related the health & wellbeing of the private client, while adhering to Six Nations Health Services and Six Nations Grand River’s policies and procedures established by Six Nations Elected Council for HCC program.
KEY DUTIES & RESPONSIBILITIES:
- Technical Functions:
- Functioning in accordance with current standards, policies and procedures and practices of Six Nations Home & Community Care for Personal Support Workers.
- Responsible for skill improvement and self-growth by attending orientation and all mandatory in-services.
- Providing a stable, safe environment while maintaining the Private Client’s activities of daily living by:
- Assisting with personal care – bathing, dressing, toileting, feeding and mobility assistance; To complete any other task delegated by Private Client that has been previously established by a Registered Nurse in accordance with the College of Nurses of Ontario Guidelines for Decision Making
- Performing a variety of household duties – light housekeeping, laundry, planning nutritional meals including special diets as requested by Private Client.

- Communications Functions:
- Demonstrates a caring attitude toward the client, family members, and other team members and for the position itself.
- Demonstrates confidence as a Personal Support Worker
- The safety of the client is maintained by carrying out duties according to Health and Safety Standards for clients, for self and other personnel.
- Administrative Functions:
- Maintaining proper use of client charts
- Observing and reporting pertinent data related to the client or their environment to the HCC Case Manager.
- Reporting and discussing any requests for changes in the treatment plan by the Private Client to the HCC Case Manager.
- Providing instruction in activities of daily living as directed by the treatment plan.
- Cooperating with fellow Private PSWs to deliver Private Client care.
- Working with other health care team members (e.g. Nurses, Physiotherapists) to assist in achieving the treatment goal.
- Ensuring that all required documentation is completed for all service provision.
- Client documentation to be forwarded to Home & Community Care quarterly for medical records.
- Presents a positive image of Personal Support Services:
- Presenting self in an acceptable, professional manner
- Uniforms preferred – clean, neat and tidy in appearance
- Good personal hygiene
- Closed toe shoes – “must have indoor shoes” as per policy.
- Other Functions:
- Provides support to the Six Nations Emergency Measures Plan by ensuring awareness of the Emergency Measures Plan and assistance as instructed by the Director of Health Services.
- It is the responsibility of the employee to be aware of and adhere to all policies and procedures, including those that relate to the Private Client’s safety, staff safety and risk management.
WORKING CONDITIONS:
- Working location is at Private Client’s personal residence. Minor administrative duties can be completed offsite at Home & Community Care location.
- Works involved external contact with referring agencies, direct client contact, including clients’ families.
- Work involves moderate physical activity through homemaking service.
WORKING RELATIONSHIPS:
With the Private Client
Receives direction, guidance, discuss priority plans, while receiving instruction, from Private Client to ensure tasks are done efficiently and effectively.
With fellow Private PSW team
Promotes courtesy, cooperation and teamwork with fellow Private PSW(s).
With the Case Manager
May receive additional direction, guidance, and discuss plans, priorities; receive instruction, supervision, priorities, or interact to ensure tasks are done efficiently and effectively.
With the Other Staff/Six Nations Agencies
Promotes courtesy, cooperation and teamwork with all staff.
With the Public/Clients
Represents and promotes the Six Nations HCC program in a courteous, positive, and cooperative manner, provides information and assistance.
KNOWLEDGE AND SKILLS:
Minimum Requirements:
- PSW Diploma/Certification
- Up to date immunization record including mandatory annual influenza vaccination.
- Valid Driver’s license, access to a reliable vehicle, with adequate insurance.

- Able to work independently as well as part of a multi-disciplinary team.
- Flexible – as changes may occur within the schedule (there may be call from fellow Private PSWs or Private Client to cover a shift). Available for all hours of service as the Private PSW program operates 365 days with 24 hour service provision as required.
- Must be in good physical health – ability to bend, lift, walk, sit, carry, pull, push and kneel.
- Communicating with fellow Private PSWs regarding Private Client related issues.
- Maintaining confidentiality.
IMPACT OF ERROR:
Errors in judgement and in the conduct of duties could lead to loss credibility, poor public relation, confusion, duplication of effort and misinformation being given to the Director of Health Services, Human Services Committee, Six Nations Elected Council, Government Agencies and the public.
CONTROL:
Guiding principles set by Health Services Department and Six Nations Elected Council. Works within the administrative policies and procedures established by the Six Nations Elected Council for the Health Services Department and other legislation provided by the respective governments.
