Developmental Services, Youth Services, Adult Counselling Services, and several Early Learning Intervention & Care services including 
EarlyON Child & Family Centres and the Community Action Program for Children. 
H-N REACH serves as the Lead Agency for Haldimand Norfolk Children’s Mental Health Services, Coordinated Service Planning and the 
Student Nutrition Program in the Hamilton/Niagara Region. Through our Contact Unit, the agency serves as the single point of access for 
a variety of children’s services. 
H-N REACH is recognized as a local leader in community education, advocacy, inter-sector collaboration and Community Service 
Planning.
Early Childhood Intervention Programs Unit- Existing Vacancy
OUTREACH WORKER - HEALTHY MOMS EATING WELL FOR 2 Ref. #2026-03
-Part-time, Permanent, Salaried
-28 hours per week (0.8 f.t.e.)
Salary*: Step 1 $39,927 - Step 2 $44,917 - Step 3 $49,909
 *Salary is based on a full-time (1.0 FTE) rate and will be prorated for this position.

Responsibilities:
• Work in collaboration with the Young Parents Outreach Worker, Healthy Babies/Healthy Children 
Family Home Visitor, contract Dietitian, partner agencies and community resources.
• Provide food supplements and prenatal vitamins to participants, as needed and the maintenance 
of proof regarding participant use.
• Coordinate individual assessments with Dietitian as well as ongoing consultation regarding 
participant nutrition.
• Establish connections with Public Health Nurses and other community agencies to provide 
program information, access referrals, and identify ongoing areas of need.
• Assist the parent in the assessing of their infant’s healthy development and in assessing required 
community services
Qualifications: 
• Successful completion of a University degree (social, education or medical sciences) from an 
institution of recognized standing or an acceptable equivalent. Focus of studies in Nutrition.
• 3-5 years’ experience working with children, with woman and issues pertaining to pregnancy, 
nutrition and parenting.
• Ability to work independently and as a team member
• Excellent communication skills 
• Vulnerable Sector Record Check (includes Criminal Record & Judicial Matters Check) and 
Children’s Aid Society Declaration/Release
• Possession of a valid driver’s licence, $2 million vehicle liability insurance, clear Driver Abstract 
and use of a vehicle.
