JOB SUMMARY: The Manager of Home and Community Care reports to and works under the direction and supervision of the Senior Manager, Primary and Clinical Care.
To assist the Department of Well-Being in the performance of statutory, operational and advisory duties related to the Home and Community Care Program and to provide leadership and operational oversight for assigned Home and Community Care programs, services and staff.
The individual will assist in planning, implementing, coordinating, developing and maintaining Home and Community Care programs and services within the policies and procedures established by the Six Nations of the Grand River Elected Council. The Manager is responsible for supporting effective day-to-day service delivery, staff supervision, program quality, client safety and the ongoing development and evaluation of assigned services to meet the needs of the Six Nations community.
