JOB SUMMARY: The Manager of Home and Community Care reports to and works under the direction and supervision of the Portfolio Lead.

 

To assist the Senior Leadership Team in the performance of statutory, operational and advisory duties related to the Home and Community Care Program. The individual will assist in planning, implementing; coordinating and developing; and maintaining various programs and managing services for Home and Community Care within the policies and procedures established by the Six Nations of the Grand River Elected Council for the Six Nations Home and Community Care services.

BASIC QUALIFICATIONS:

  • Bachelor’s Degree and three years of related experience in a health setting

OR

  • College Diploma and three years of related experience including supervision of support staff in a health setting
  • Knowledge of end of life care
  • Demonstrated experience working in an inter-disciplinary team
  • High level of computer skills – Microsoft Office – MS Word and Excel Other Related Skills:
  • Strong project management skills and experience, including the ability to define and meet project purpose and objectives
  • Demonstrated strong working knowledge and understanding of end of life care, palliative care and health care system
  • Highly developed analytical, problem solving and negotiating skills
  • Ability to exercise discretion, judgment and work independently with a high degree of initiative and creativity in problem solving
  • Excellent written and oral communication, presentation and interpersonal skills and the ability to work cooperatively with others
  • Excellent organizational skills

POSITION DESCRIPTION – HEALTH SERVICES

 

POSITION TITLE: Manager of Home and Community Care

 

REPORTING RELATIONSHIP:

 

Reports to and works under the direction and supervision of the Portfolio Lead.

 

PURPOSE & SCOPE OF THE POSITION:

 

To assist the Senior Leadership Team in the performance of statutory, operational and advisory duties related to the Home and Community Care Program.

 

The individual will assist in planning, implementing; coordinating and developing; and maintaining various programs and managing services for Home and Community Care within the policies and procedures established by the Six Nations of the Grand River Elected Council for the Six Nations Home and Community Care services.

 

KEY DUTIES & RESPONSIBILITIES:

 

  1. Technical Functions:
  • Approve tenders for the purchase of equipment, materials and services; examine tender proposals to ensure that they meet the specified needs and recommended actions.
  • Program planning, development and evaluation.
  • Ensures programs are developed to meet the needs of the community through review, modification and evaluation.
  • Prepares program reports for the department, Central Administration, Six Nations of the Grand River Elected Council and funding agencies as required.
  • Support the identification and acquisition of funds for capital and operational needs of the program
  • Responsible for being familiar with funding agreements, eligible expenses and reporting requirements.
  • Monitors program budgets to ensure balancing of statements and to ensure programs are operating within budget.
  • Identify and support provision of necessary training for team members.
  • Monitors day to day operations and supports team members as required.
  • Coordinate the delivery of services provided by providing supervision/direction/leadership and advice to the staff as required to ensure efficient and coordinated service delivery to clients by all service components.
  • Supervise staff and evaluate employee performance consistent with the policies and administrative processes of the Home and Community Care program.

 

  1. Communications Functions:
  • Support community awareness of services offered.
  • Ensure community access to reliable educational materials and supports related to all program areas.
  • Participate in committees and working groups as required.
  • Work collaboratively to identify and resolve community service issues.
  • Advocate for Six Nations community needs at every level of inter-agency communication.
  • Establish and maintain communication with agencies external to Six Nations of the Grand River for the purpose of awareness and potential program expansion.
  • Foster and maintain positive relations with Six Nations community services.
  • Provide leadership in the identification, adoption and promotion of programs and services to enhance the health and well-being of the people of Six Nations community.

 

  1. Administrative Functions:
  • Support financial processes within given authority and maintain documentation as required.
  • Monitor the spending of budgets for Home and Community Care programs within administrative and financial guidelines.
  • Document orientation of all new employees and student placements.
  • Conduct annual performance evaluations for all direct report team members.
  • Review and maintain program policies as required.
  • Identify and manage projects as required to support the development of the program.
  • Adhere to organizational policies and procedures.
  • Assist with planning, developing and implementing the program work plan consistent with community, department and council guiding documents, mission and principles.

 

  1. Other Functions:
  • Assist in the research and preparation of statistical, financial, policy and other reports required by the Senior Leadership Team.
  • Monitor adherence to occupational health and safety procedures.
  • Monitor and report issues and concern related to the operation of Six Nations health services facilities, as appropriate. Revised January 2024
  • Performs other job related duties as may reasonably be required by the Senior Leadership Team.
  • Provides support for the Six Nations Emergency Measures Plan by ensuring awareness of the Emergency Measures Plan and assistance as instructed by the Senior Leadership Team.

WORKING CONDITIONS:

 

  • Work requires extensive interaction with public and is subject to deadlines,

interruptions, mental stress and unscheduled hours of work.

  • This position requires inside work and requires some travel
  • Ability to take direction, prioritize and work independently with many demands

and time restraints.

Represents and promotes Six Nations interests relative to Health Services; maintains awareness of legislative policy and program changes; seeks to develop sound, professional working relationships.

Errors in judgement and in the conduct of duties could lead to loss credibility, poor public relation, confusion, duplication of effort and misinformation being given to the Director of Health Services, Human Services Committee, Six Nations Elected Council, Government Agencies and the public.
