JOB SUMMARY:  

The Indigenous Transition Facilitator plays a vital role in supporting clients through transitions in healthcare, including entering care and being discharged back home. This position helps clients access healthcare services, advocates for equitable and culturally safe care, and guides clients through the healthcare system, as well as community, provincial, and federal programs and supports.

Working as a collaborative member of both the Oneida Health Centre team and external healthcare partners, the Indigenous Transition Facilitator builds relationships with both Indigenous and non-Indigenous healthcare providers. The role addresses service gaps by facilitating connections to various services, including (but not limited to) mental health and addictions, chronic disease management, primary care, acute care, and palliative or end-of-life care.

The Indigenous Transition Facilitator will actively promote the vision and mission of the Home and Community Care program and contribute to community development, program planning, resource utilization, quality assurance, training, evaluation, and policy development. Collaboration and fostering internal partnerships within the Oneida Health Department are central to this role.

EDUCATION:

1. A Diploma in Registered Practical Nursing (RPN).
2. Current member, in good standing, with the College of Nurses of Ontario.
3. Must have valid First Aid/CPR certificate.

EXPERIENCE:

1. Three (3) years direct experience in delivery and/or coordination of home care services.
2. Demonstrated clinical knowledge and experience in home care case management and/or community-based home care service delivery.
3. A combination of education and work experience pertaining to community-based health care services may be considered.
