Hours: 37.5 hours per week, Monday to Friday, 8:30a.m. to 4:30p.m., must be flexible to align work schedule with program requirements which will include some evenings, weekends, and statutory holidays, must fulfill on-call services as required

JOB SUMMARY:  

The Home and Community Care Nurse is responsible for providing comprehensive care and case management to clients within the Home and Community Care Program. This role involves delivering health education, promoting wellness, and offering support to both clients and their families in line with nursing standards of practice. The nurse works within their professional scope to ensure the health and wellbeing of clients, collaborating closely with other healthcare providers, both within the Oneida Health Centre and external organizations.

In addition to primary care, the nurse actively participates in the development, implementation, monitoring, and evaluation of care services tailored to the needs of individuals and families. The position requires a commitment to the philosophy, goals, and objectives of the Home and Community Care Program. The nurse will adhere to all relevant organizational policies, procedures, and legislation as an employee of the Oneida Nation of the Thames.

 

EDUCATION:

  1. A Diploma in Registered Practical Nursing (RPN).
  2. Current member, in good standing, with the College of Nurses of Ontario.
  3. Must have valid First Aid/CPR certificate.

 

EXPERIENCE:

  1. A minimum of two (2) years employment experience in nursing, including in a primary care or community setting.
  2. Experience working in First Nation communities is considered an asset.

REQUIREMENTS PRIOR TO STARTING EMPLOYMENT:
