POSITION: Health Case Manager – Mino Kaanjigoowin

SHIFT: Monday to Friday 8am-4pm

SALARY: $50,000

Na-Me-Res (Native Men’s Residence) located in Toronto, is a diverse, multi-service organization. Through a culture-based approach that addresses the holistic needs of its clients, Na-Me-Res’ mission is to provide temporary, transitional and permanent housing to Aboriginal men experiencing homelessness in Toronto, while providing outreach and support services to the broader Aboriginal homeless population.

MAIN FUNCTION: The Health Case Manager is a member of the Mino Kaanjigoowiin team, a housing First intensive case management team working with Indigenous clients who experience chronic mental health and addiction issues. This program is a part of the Na-Me-Res (Native Men’s Residence) range of programs working with clients experiencing homelessness.


  • Complete thorough intake, assessments, and develop case plans with clients from a recovery-oriented approach
  • Collaborate with members of the interdisciplinary team to plan, implement, coordinate and evaluate client care
  • Provide appropriate referrals to address clients physical, mental, emotional and spiritual needs
  • Ensure that program delivery is consistent with Indigenous culture and traditions
  • Prevents and/or resolves developing crises to avoid unnecessary use of emergency rooms and re-hospitalization
  • Assures the maintenance of case management records with appropriate reviews and updating of client care plans
  • Prepare and facilitate health promotion activities
  • Follow clients from homelessness to housing providing follow up supports as required
  • Liase and advocate on behalf of clients with community agencies, including health providers
  • Other related duties and identified and required by the Manager


  • Post-secondary education in Social Work or Nursing
  • Registered member of respective Ontario College, in good standing
  • 3-5 years experience working with people who are homeless and have mental health and addiction issues
  • Specialized knowledge and ability to navigate the health and mental health system
  • Strong capacity to manage crises and problem solve
  • Excellent interpersonal skills
  • Knowledge of Housing First Principles
  • Knowledge of Indigenous social and health related issues
  • Clear Police reference check required as a condition of employment
