Job Summary

Community Paramedicine (CP) is a model of care that has emerged in an effort to maximize efficiencies in patient care and resources. These programs provide an innovative model of care that helps to improve access to additional support services for seniors and patients with chronic health and social issues. The development and expansion of these programs allows paramedics to apply their education and skills beyond the traditional role of paramedic response.

The Ministry of Long-Term Care has recently provided funding for Community Paramedicine for Long-Term Care (CP-LTC), an innovative program to provide services to individuals in the community waiting placement for long-term care, or whom will soon be eligible for long-term care. This funding announcement has resulted in an increase of 2.5 million dollars to Oneida Nation of the Thames Community Paramedicine program. Our program will work to address systemic barriers in long-term care bed development by supporting these vulnerable individuals in their homes.

CP program delivery is defined through collaboration between our health care partners, to address their unique requirements, but generally fall under three broad categories (Assessment and Referral – follow up on known vulnerable patients within our system, Home Visits – working with family health teams, and Clinics. The model of care will include paramedics in an expanded role and expanded scope of practice under the medical delegation of a local physician, including but not limited to a palliative care scope of practice and point of care testing. As the role of the Community Paramedic expands, activities and enhancements of this role may be expanded based on identified gaps. 

Community Paramedics will work in either a 4-on, 4-off scheduling pattern or continental scheduling pattern, based on operation needs of the program. Regular rotations will begin upon the completion of training.

CP is currently not included under the definition of ambulance service in the Ambulance Act, nor is it a core legislated activity under the current mandate that governs Paramedic Services and resources.

Qualifications

  • Current qualifications as a Primary Care Paramedic (PCP) as per the Ambulance Act of Ontario.
  • Ontario Secondary School Diploma or equivalent approved by the Ministry of Education
  • Paramedic Program Diploma/Certificate
  • Base Hospital Certification.
  • Current certification in CPR (Basic Rescuer level).
  • Valid and current Ontario Driver’s (Class F minimum) Licence sufficient to operate an Ambulance.
  • Successful completion of BLS Standards Program.
  • Free of communicable disease as per the Ambulance Service Communicable Disease Standards.
  • Up-to-date immunizations as per Ambulance Act of Ontario.
  • Successful completion of all Ministry of Health and Long-Term Care (MOHLTC) Core Mandatory Training to date.
  • Vulnerable Sector Search required.

Knowledge and Experience

  • Certification as a Primary Care Paramedic or Advanced Care Paramedic in accordance with the Ontario Provincial Symptom Relief and Cardiac Arrest Medical Directives
  • Paramedic Diploma from a community college
  • AEMCA Certification

Special Conditions

  • Valid Ontario Class F Driver’s License.
  • In accordance with the Corporate Criminal Record Check Policy, the position requires the incumbent to undergo a
  • Criminal Records Check and submit a Canadian Police Clearance Certificate inclusive of a Vulnerable Sector profile.
  • Maintain PCP status as a condition of employment as it relates to ability to provide advanced training to paramedic staff.
  • An annual influenza vaccination is recommended.
  • Physical dexterity, coordination and strength, including fine and gross motor skills and a demonstrated ability to perform patient extrication, positioning, lifting and carrying techniques to rapidly access, treat, convey and manage patient conditions under frequently awkward, difficult and unpredictable environmental conditions.
  • Work is performed independently within established work procedures, department procedures and safety procedures, as well as within service policies and procedures, Base Hospital protocols, Ministry of Health Basic Life Support Standards, the Ambulance Act, the Radio Act and the Mental Health Act.

Competencies

  1. Written and verbal communication skills sufficient to perform documentation and related administration duties, as well as to relate to patients, families and other health care professionals in a calm and professional manner.
  2. Demonstrated skills in the areas of teamwork, problem solving and leadership sufficient to deal with crisis situations.
  3. Demonstrated ability to work independently with minimal direct supervision. 

Major Responsibilities

  • Liaise with primary care providers and allied health providers such as physicians, pharmacists, care coordinators
  • Conduct routine and ad hoc visits on vulnerable patients
  • Provide patient education, self-management coaching, welfare checks, post exacerbation and hospital discharge follow up support to manage their underlying condition(s)
  • Conduct physical/biometric/advanced assessments for chronic conditions
  • Provide timely reports to primary care providers to support with informed clinical decisions on patient management
  • Maintain current status and accurate records of patients
  • Enroll and monitor all rostered patients for trends and events requiring medical assistance
  • Assist with developing unique coordinated care plans
  • Prepare qualitative and quantitative reports
  • Provide general or appropriate pre-hospital emergency medical care and arrange for transport for further medical assistance as required as per the Basic Life Support Standards
  • Work under the designation of physician(s) outside of the OBHG. Expanded scopes which could include but not limited to;
    • Vaccinations- Influenza and COVID-19
    • Chronic disease management
    • Point of care testing

Mandatory Requirements

  • Certification as a Paramedic in accordance with the Ontario Provincial Symptom Relief and Cardiac Arrest Medical Directives
  • Must hold all qualifications as per the Ambulance Act
  • Must possess and maintain a valid Class “F” Ontario Driver’s License
  • Must be in compliance with all immunization standards, as per the Ambulance Service Communicable Disease Standard Version 2.0
  • Excellent patient advocacy skills and community engagement
  • Excellent communication and public speaking skills
  • Computer literacy utilizing MS Office and iMedic applications and proficiency in word processing; ability to quickly learn new data repository systems, electronic medical record platforms etc.
  • Knowledge and experience in adult education concepts with the ability to modify approaches for different learning styles
  • Demonstrate understanding of chronic conditions (CHF, COPD, DM) and management
  • Demonstrate understanding of public health/social issues and resources
  • Demonstrated the ability to maintain a high level of patient satisfaction
  • Demonstrate the ability to foster a positive and co-operative working relationship with team members, allied services, and other agencies and the public
  • Candidates must be willing and available to participate in a professional development program
  • Candidates must be accommodating to a dynamic rotation schedule as programming requires

Preferred Requirements

  • Prior experience and knowledge related to Community Paramedicine
  • Relevant continued education learning (i.e., Community Paramedicine professional development course, LEAP Paramedic, adult education etc.)
  • Customer Service/Stakeholder relationship skills
