POSITION: Big Eya Wah Anduyaun Follow-Up Worker (6-month contract)

LOCATION: Outreach

HOURS: 8am – 4pm Monday to Friday

Na-Me-Res (Native Men’s Residence), located in Toronto, is a diverse, multi-service organization.  Our Mission Statement is: Through a culture-based approach that addresses the holistic needs of its clients, Na-Me-Res’ mission is to provide temporary, transitional and permanent housing to Indigenous men experiencing homelessness in Toronto, while providing outreach and support services to the broader Indigenous homeless population.

 

MAIN FUNCTION:

The Follow-Up worker will provide newly housed people who have been experiencing long term homelessness with the resources and support required to enhance the quality of their life, hence optimizing the likelihood of their remaining successfully housed.

 

DUTIES AND RESPONSIBILITIES:

  • Conduct client assessments and develop case management plan
  • Receive referrals, review case summaries, and create client files
  • Participate in joint meeting with client and Housing Worker
  • Develop short- and long-term goals to maximize a client’s chances of remaining successfully housed
  • Assess client’s health and identify furnishing needs
  • Meet with client on a regular basis to monitor progress toward goals
  • Assess and revise case plans as required
  • Provide tenant/housing provider mediation service as required, assist with eviction prevention and any other interventions which may be necessary. If unsuccessful, assist with rehousing client.
  • Participate in monthly case conferences
  • Maintain up-to-date file on each client including case notes and referrals
  • Provide all reports to City of Toronto and Na-Me-Res as required
  • Other duties as identified by the Manager of Outreach Services

 

EDUCATION, EXPERIENCE, & SKILLS:

  • Several years of related work experience
  • A valid Ontario Driver’s License and clean driving abstract is an asset
  • Must pass Police check
  • Knowledge and ability to work with computers
  • Ability to write and communicate clearly and effectively
  • Ability to prioritize and manage multiple jobs effectively
  • Successful experience working with a diverse population
  • Knowledge of Native traditions and culture is an asset
  • A degree or diploma in social services or other related discipline or equivalent

SALARY: $28.85 hourly
