Position: Aboriginal Healthy Babies Healthy Children Coordinator
Start date: ASAP following hire
Hours of Work: 32 hours per week
The successful candidate will be employed by the Fort Erie Native Cultural Centre Incorporated, responsible to the policies and directions determined by the Board of Directors and under the daily supervision of the Executive Director or his/her delegate.
Primary Objective:
- Ensure that urban Indigenous families and children (prenatal to age six) have access to effective and consistent early intervention services; and provide the best opportunities for healthy child development through home visiting, service coordination, parenting groups, cultural teachings, and traditions.
Education/Work Experience
- Relevant college or university level accreditation or equivalent work experience.
Key Responsibilities and Duties
- Home visits to establish and develop a plan of care
- Ongoing review of progress and support requirements of the family support plan and adjust accordingly
- Identifying additional support/resources that may be required and assisting in appropriate referrals, screening services and support services
- Advocating on behalf of families and their children ensuring treatment is received in a culturally sensitive manner
- Establish partnerships with nurses, Elders, social services workers, midwives, Early Years, Brighter Futures, community health representative, community wellness worker, etc.
- Initiate/participate in case management/case conferencing when needed.
- To work toward reducing and preventing family violence and improving Indigenous health by increasing awareness of specific issues, risk or concerns in the group or the community.
- To foster healing and empowering individuals, families and community to achieve and enjoy a healthy and balanced life.
- Self-motivator with ability to work independently and in a team environment.
- Exceptional oral and written communication skills.
KNOWLEDGE
- Takes personal responsibility to increase sensitivity, awareness and implementation of FENCC including foundational teachings and organizational practices in both professional conduct and work-related deliverables.
- Facilitated or coordinated by the AHBHC worker, a program or service that involves the specific use of traditional Indigenous knowledge, practices, medicines, teachings or services to achieve the objectives of the activity or event.
- Implement traditional healing circles and cultural teaching circles such as Medicinal teachings, Seven Grandfather teaching, traditional parenting circle.
- High proficiency in Excel and other MS Office applications
- Must have current, clear, vulnerable sector police check.
- Must have a clean driver’s abstract and current driver’s license.
