JOB SUMMARY: The Community Support Worker reports to and works under the direction and supervision of the Supervisor of Community Support Services Program, and in accordance with Health Services. The Community Support Worker is to implement client service plans to deliver high-quality home and community care services to community seniors, convalescence, and disabled persons as required; maintaining/reviewing assigned case files; planning weekly shopping trips, social activities and completion of weekly reports/statistics as required; and for the performance of other related duties as required by the Supervisor.

BASIC QUALIFICATIONS:

  • Must have a PSW Diploma or Certificate
  • Must have a valid class G driver’s license and insured vehicle
  • May need to obtain a class F license
  • Certification in First Aid and CPR
  • Must be in good health – will be required to submit a favourable medical health assessment
  • Will be willing to work flexible hours
  • Good knowledge of the Six Nations Community, services available in area, and high sensitive to Six Nations Culture
  • Must pass criminal record check
  • Will be knowledgeable about Canada’s Food Guide
  • Must relate effectively to the elderly, and disabled person
  • Organize work, communicate clearly and effectively, both written and verbal
  • Work independently and work cooperatively with others

POSITION DESCRIPTION – HEALTH SERVICES

POSITION TITLE: Community Support Worker

REPORTING RELATIONSHIP:

Reports to and works under the direction and supervision of the Supervisor of Community Support Services program, and in accordance with Health Services.

PURPOSE & SCOPE OF THE POSITION:

To implement client service plans to deliver high quality home and community cares services to community seniors, convalescence, and disabled persons as required; maintaining/reviewing assigned case files; planning weekly shopping trips, social activities and completion of weekly reports/statistics as required; and for the performance of other related duties as required by the Supervisor.

KEY DUTIES & RESPONSIBILITIES:

  1. Technical Functions:
  • Implementing client service plans to deliver high quality home and community care services to community seniors, convalescence, and disabled persons.
  • Planning weekly shopping trips, social activities and completion of weekly reports/statistics as required.
  • Delivering the planned on-going or brief service(s) to the client who may include Meals-on-Wheels, Generic Transportation, Homemaking, Friendly Visiting, Telephone Reassurance, and Diner’s Club/Wheels to Meals/Congregate Dining (Sr. Day).
  • Providing transportation to the frail elderly to shop and to participate in various social activities, escorting during the trip due to shot or long-term physical limitations.
  1. Communications Functions:
  • Working with the Supervisor to determine case priority based on the established Home and Community Care/Community Support Services policy and procedures, Mission Statement and Council policies.
  • Attend monthly staff meeting.
  1. Administrative Functions :
    • Ensuring that assessment of work required and actual work performed is duly documented and placed on client file.
    • Ensuring service plan is approved by the Supervisor.
    • Periodically reviewing case files and recommending necessary changes, getting the Supervisor’s approval for changes and amending service plans as required.
    • Following established reporting procedures as laid out by the program, completing necessary monthly/weekly report forms and ensuring relevant statistics are maintained as required.
  1. Other Functions:
    • Other duties as assigned or deemed necessary as relates to the position.
    • Provides support to the Six Nations Emergency Measures Plan by ensuring awareness of the Emergency Measures Plan and assistance as instructed by the Director of Health Services.
    • It is the responsibility of the employee to be aware of and adhere to all policies and procedures, including those that relate to client safety, staff safety and risk management.

WORKING CONDITIONS:

  • Work involves external contact with referring agencies, direct client contact, frequently in the client’s homes.
  • Work involves moderate physical activity through homemaking service, Meals on Wheels delivery etc.
  • Work will require some travel in the community as required using own transportation.
  • Work required rotational assignments in office as scheduled; with majority of time spent providing service out in the community.

Receives direction,   guidance and discusses plans, priorities or interacts to ensure tasks are done efficiently and effectively; received instructions supervision.

With the Other Staff/Six Nations Agencies

Promotes courtesy, co-operation and teamwork with all staff.

With Public/Clients

Represents and promotes the Six Nations Community Support Services program in a courteous, positive, and cooperative manner, provides information and assistance.

With External Agencies

Represents and promotes Six Nations interest related to the Community Support Services Program; maintains a general awareness of legislation, policy and program changes, seeks to develop sound professional working relationships.

IMPACT OF ERROR:

Errors in judgement and in the conduct of duties could lead to loss of credibility, poor public relation, confusion, duplication of effort and misinformation being given to the Director of Health Services, Human Services Committee, Six Nations Elected Council, Government Agencies and the public.

CONTROL:

Guiding principles set by Healthy Services Department and Six Nations Elected Council. Works within the administrative policies and procedures established by the  Six Nations Elected Council for the Health Services Department and other legislation provided by the respective governments.
