INTRODUCTION:

De dwa da dehs nye>s Aboriginal Health Centre’s Mental Health Department is seeking 1 Part-Time ENS Coordinated Service Planner based in St. Catharines, with travel throughout the Niagara region. The Extensive Needs Services program launched in April 2023 at hospitals, including Hamilton Health Sciences’ McMaster Children’s Hospital, and is focused on providing support to children and youth with urgent and complex needs, and their families. As a community partner of this initiative in the Niagara region, this service aims to provide connections to supports for children and youth with challenging or interfering behaviors, whose needs remain unmet after already accessing services, and have a complex profile of needs such as neurodevelopmental conditions, ABIs, mental health conditions, or co-occurring chronic physician health conditions. The successful candidate will provide one-on-one case management and social support services to eligible children and youth ages six 6 to 24, and their families.


REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:

• Knowledge and ability to integrate wholistic, strength, trauma-informed, and evidence-based practice into the assessment,  planning, treatment, and monitoring of client care plans.
• Knowledge of Indigenous cultures, customs, and traditions is an asset.
• Familiarity with the OCSWSSW Code of Ethics & Standards of Practice, CFSA, Youth Justice Act, Mental Health Act, Duty  to Report, and any other act as it pertains to the client(s) circumstances
• Knowledge of the history of colonization and intergenerational traumas impacting the health and wellness of Indigenous  Peoples
• Is skilled in group facilitation, conflict resolution, de-escalation, and grounding techniques.
• Familiarity with child and youth counselling models, techniques, modalities, and approaches.
• Knowledge &/or Experience working with Children’s Aid Societies
• Demonstrated ability to work independently and in a team environment.
• Knowledge of tools, systems, and databases used in client services delivery and case management.
• Knowledge and understanding of pathways of care for children and youth with complex needs.
• Possess self-awareness and engage in self-care techniques.
• Possess excellent time management, organization, problem-solving, and communication skills


STATEMENT OF QUALIFICATIONS:

• Post-secondary degree in social work, psychology, &/or concurrent disorders or related field – Master’s Degree Preferred

In addition to requirements for the position, the ideal candidate would also possess some of the following qualifications:

• 1+ years’ experience working with individuals with complex needs
• Experience working with and extensive knowledge of Indigenous Peoples' language, culture, customs, and traditions.
